Cost
The cost of the plan is $17.25 per month and will be deducted on a post-tax basis from your paycheck.
How to Enroll
Once enrolled, you will be required to remain in the plan for the full benefit plan year.
If you are currently enrolled and no longer wish to be, you must actively waive coverage during Open Enrollment.
