Who Can Enroll?

Full-Time Employees

Full-time employees who have completed 30 days of active employment are eligible for benefits.

Part-Time Employees

Part-time employees who work an average of 30 hours or more each week over 12 consecutive months, are eligible for benefits.

Dependents

If you elect coverage, you may also enroll your eligible dependents.

Eligible dependents are:

• Your legal spouse;
• Your children under the age of 26; and/or
• Your children over age 26 who are not able to support themselves due to a physical or mental disability.

If you enroll dependents, you will need to provide documentation such as a birth certificate, a marriage certificate, etc., within 30 days of enrollment. Dependents will appear as pending coverage until the appropriate documentation is received.
